Form 1S. Admission Form

For new students who wants to study with us, kindly download and print this form. Fill it with the needed information, and then bring it with you to Kanlayanee English Program's Office. Give it to the secretary with your other documents.

Form 2S. Waiver Form

For students who are planning to attend activities and trips outside of school, please download and print this form, fill it with necessary information, have it read and signed by your parents or guardian, and then submit it to the EP secretary.

Form 4S. Passport Application Form

For students who are joining a tour to other countries, you need to get a passport if you do not have one yet. Download and print this form.

Form 3S. Excuse Letter Template

For students who wish to be excused from their classes due to valid reasons, please have this form downloaded and printed. Supply the required details, and then hand it over to your teacher/s.
